Mid-Face Volume Loss in Cheongdam & Apgujeong, Seoul

Mid-face volume loss is deflation of the deep cheek fat and bony support beneath the eyes and cheeks, distinct from sagging. At Doje Clinic in Cheongdam and Apgujeong, Seoul, Dr. Byun Jeik restores structural support with retaining-ligament filler, Radiesse or collagen boosters within the Mid-Face Program, using only as much as needed.

Deflation is not the same as sagging

The mid-face — 중안부 in Korean — ages in two ways at once. Tissue can descend, sliding down as ligaments loosen, and it can deflate, losing the volume that once held it out. The two look similar in photographs but need different treatment, so separating them is the first step.

Volume loss in the mid-face comes from several layers:

  • Deep fat pads. The deep medial cheek fat and the fat beneath the muscle around the lower eye shrink with age. These pads act as a platform for the more superficial tissue; when they deflate, the front of the cheek flattens.
  • Bone. The upper jaw gradually recedes, particularly beside the nose, and the lower rim of the eye socket widens. The skeleton that projects the cheek forward provides less support.
  • Skin. Thinner skin with less collagen drapes more closely over these changes and reveals them.
  • Accelerators. Weight loss, endurance training, smoking and genetics can bring mid-face deflation forward by years.

Visible signs include a flatter front of the cheek, a longer junction between lower lid and cheek, a deeper base to the smile line, and hollows below the eye.

How Doje Clinic assesses the mid-face

In a 1:1 consultation, Dr. Byun Jeik — named one of Korea's 22 filler experts by Medical Times and a Merz Advisory Board member — uses a simple but revealing comparison: the face upright and the face reclined. When you lie back, descended tissue tends to fall back towards where it came from, while true volume loss remains visible. What stays hollow in the reclined position is what needs support.

  • Where volume is missing: medial cheek, lateral cheek, beside the nose or below the eye.
  • How much descent accompanies the deflation.
  • Skin thickness and elasticity, which affect product choice; Mark-View 3D skin analysis adds objective data.
  • Your facial proportions and the look you want to keep.

The aim is to restore a few key support points so the face regains its shape, not to fill each hollow until it disappears.

Options from mild to advanced

Early, diffuse deflation

When volume loss is subtle and spread out, collagen boosters such as GOURI or Re2O can improve firmness and fullness gradually, without placing a defined volume of product. Results build over weeks to months.

Loss of key support points

Where specific anchor areas have deflated, retaining-ligament filler is placed deep, close to the bone, at the points where ligaments hold the mid-face up. Small, strategic amounts can restore shape and indirectly soften the tear trough and smile line. A firmer structural filler may be used for projection where bone support has receded.

Volume loss with thinner skin

Radiesse, a calcium hydroxylapatite filler with a biostimulatory effect, can provide immediate support while encouraging the skin's own collagen, which suits some patients with thinner skin or those who prefer a gradual, firm result.

Deflation with descent

When both are present, filling alone tends to make the face look heavy. The Doje signature mid-face thread lift repositions tissue first, so that less volume is needed afterwards.

Which Doje program fits

The Mid-Face Program is built for mid-face sagging, volume loss and elasticity: signature mid-face thread lift, retaining-ligament filler and nasolabial-line filler, with Radiesse, Ultherapy Prime, Softwave and GOURI/Re2O as options. For volume loss without much descent, the filler and booster elements usually lead.

When a surgical opinion is more appropriate

If you would prefer a longer-lasting volume solution using your own tissue, fat grafting is a surgical option that requires assessment by a qualified surgeon. When marked descent and excess skin accompany deflation, a mid-face lift or facelift may be more appropriate than layering filler. Dr. Byun will discuss these honestly.

Cost

Fees depend on the products, amounts and whether lifting is needed, and are quoted after the 1:1 consultation. Ask for a written quote; Korean non-covered (비급여) aesthetic fees are set per plan.

Frequently asked questions

How is mid-face volume loss different from sagging?

Volume loss is deflation of fat and bone support; sagging is tissue moving downward. Most people have some of both, and the balance decides the treatment.

Will filler make my face look puffy?

It can if too much is used or if filler is added to descended tissue. Placing small amounts at key support points avoids this.

Radiesse or hyaluronic acid filler?

Hyaluronic acid filler is adjustable and suits most areas; Radiesse adds a collagen-stimulating effect. The choice depends on skin, area and goals.

Can collagen boosters replace lost volume?

They improve firmness and subtle fullness over time but do not replace significant structural volume, which needs filler.

How long do results last?

It varies with the product, area and individual metabolism; deep mid-face filler often lasts relatively long.

What is the downtime?

Usually short: mild swelling, tenderness or a small bruise for several days is common.
