Two kinds of flat cheek
When people describe their cheeks as flat, they usually mean the front of the cheek — 앞광대 in Korean — the soft, rounded area below the eye that catches light when you smile. Its absence can make a face look tired, older or less expressive. But flatness has two quite different origins, and they need different plans.
- Structural flatness. Some people have a flatter mid-face by nature, with less forward projection of the upper jaw and cheekbone. This is a common variation in facial structure, present from early adulthood and not a sign of ageing. The goal here is gentle enhancement, in proportion with the nose, lips and chin.
- Age-related flatness. In others, the cheek was once fuller. The deep medial cheek fat shrinks, the bone beneath the eye and beside the nose gradually recedes, and the superficial fat descends. The front of the cheek flattens while the folds below deepen.
A common complication is width. Many faces with flat front cheeks have relatively prominent side cheekbones; adding volume in the wrong place makes the face look wider and heavier rather than more youthful. Placement, not quantity, decides the outcome.
How Doje Clinic assesses flat cheeks
In a 1:1 consultation, Dr. Byun Jeik — a Merz Advisory Board member named one of Korea's 22 filler experts by Medical Times — studies the cheek from the front, in three-quarter view and in profile, at rest and smiling. He considers:
- whether flatness is structural, age-related or both;
- the width of the face at the side cheekbones, so that projection is added forward rather than outward;
- whether the cheek has descended and should be lifted before any volume is placed;
- skin thickness, which affects product choice;
- your proportions and how much change feels right for you.
Where skin quality is part of the picture, Mark-View 3D skin analysis adds objective data.

Options from mild to advanced
Mild flatness with good skin
A small amount of firm structural filler placed deep on the bone at the front of the cheek creates a soft point of projection and highlight. Placement is planned to bring the cheek forward rather than outward, so the face does not look broader.
Flatness with thinner skin
Radiesse provides firm support with a collagen-stimulating effect, which can suit thinner skin or those who prefer a gradual, structural result. The choice between hyaluronic acid filler and Radiesse depends on skin, area and goals.
Age-related flattening
When the cheek has deflated, retaining-ligament filler restores the support points that hold the mid-face up, often improving the area below the eye and the smile line at the same time.
Flatness with descent
If the cheek fat has slid downward, it should be repositioned before volume is added. The Doje signature mid-face thread lift lifts the descended tissue back towards the front of the cheek, so that much of the original fullness returns and less filler is needed.
Which Doje program fits
The Mid-Face Program brings these elements together: signature mid-face thread lift, retaining-ligament filler and nasolabial-line filler, with Radiesse, Ultherapy Prime, Softwave and GOURI/Re2O as options. For purely structural flatness, a single filler treatment may be all that is needed.
When a surgical opinion is more appropriate
If you would like a substantial, permanent change in cheek projection or facial skeleton, cheek implants or facial bone surgery, assessed by a qualified surgeon, are the surgical routes. If age-related descent is extensive with excess skin, a mid-face or facelift may be more appropriate than filler. Dr. Byun will explain where non-surgical options reach their limits.
Cost
Fees depend on the product, amount and whether lifting is needed, and are quoted after the 1:1 consultation. Ask for a written quote; Korean non-covered (비급여) aesthetic fees are set per plan.

Frequently asked questions
Are my flat cheeks natural or caused by ageing?
If your cheeks have always been flat, it is likely structural. If they were fuller in earlier photographs, deflation or descent is involved. Many people have both.
Will cheek filler make my face look wider?
Not when it is placed at the front of the cheek to project forward. Adding volume over the side cheekbones is what widens the face, so placement is planned carefully.
Hyaluronic acid filler or Radiesse?
Both can provide structural support. Hyaluronic acid is adjustable; Radiesse adds a collagen-stimulating effect. The choice depends on your skin and goals.
Can a thread lift add volume to flat cheeks?
Threads do not add volume, but by repositioning descended tissue they restore fullness to the front of the cheek in age-related flattening.
How long do results last?
It varies with the product, depth and individual metabolism; deep cheek filler often lasts relatively long.
What is the downtime?
Usually short: mild swelling and tenderness for a few days are common.
