Where facial volume goes
Facial fat is not one sheet; it is arranged in separate compartments, some just under the skin and others deep against bone. These compartments do not age at the same rate. Deep pads in the cheek and around the eyes tend to deflate earlier, while some superficial compartments in the lower face may keep or even gain fat. The result is not just a thinner face but a redistributed one: hollow above, heavier below.
Several processes combine (얼굴 볼륨 감소 in Korean):
- Fat compartment shrinkage. The temples, under-eye area and front of the cheek are typically first to lose fullness.
- Bone remodelling. The eye sockets widen, the upper jaw recedes and the jawline in front of the jowl thins, reducing the scaffold that projects soft tissue.
- Skin thinning. Less collagen means skin that drapes over every contour rather than smoothing it.
- Weight loss. Significant or rapid weight loss — through diet, endurance sport, illness or weight-loss medication — can reveal hollows that would otherwise take years to appear.
Genetics also play a role: some people have naturally lean faces and notice hollowing in their thirties, while others keep facial fullness well into later life.
How Doje Clinic maps volume loss
Treating volume loss well means deciding where to restore, not just how much. In a 1:1 consultation, Dr. Byun Jeik — a Merz Advisory Board member, Belotero Expert Committee member and Merz Golden Record Award recipient — maps the face region by region and ranks the areas whose loss most changes your overall shape. Typically these are structural points: temples, lateral and front cheek, the pre-jowl area and the chin.
- Which areas have deflated, and which still have good volume?
- Is there descent that should be lifted before any volume is added?
- How thick is the skin, and would a collagen-stimulating product suit it?
- What has changed recently — for example, weight loss — and is your weight now stable?
Mark-View 3D skin analysis adds objective skin data. The plan that follows usually restores a few foundations first and refines later, rather than filling lines one by one.

Options from mild to advanced
Early, diffuse loss
When the face is just beginning to look thinner, collagen boosters such as GOURI or Re2O can improve firmness and subtle fullness gradually. Skin boosters improve the skin's quality so that it reflects light better and looks less drawn.
Defined hollows
Hyaluronic acid filler restores volume precisely. Doje distinguishes between soft fillers for delicate, mobile areas such as under the eyes and beside the mouth, and structural fillers for areas that need support and projection, such as the temples, cheekbones and chin. Placement at structural points often lifts and reshapes neighbouring areas too.
Thin skin over deflated areas
Radiesse offers support with a collagen-stimulating effect and may suit people with thinner skin who want a firm, gradual result.
Volume loss with sagging
If descent accompanies deflation, the Doje signature thread lift repositions tissue first, so that much less volume is needed afterwards. This is the main safeguard against the overfilled look.
Staging
Larger volume restoration is often planned over more than one session, allowing tissue to settle and results to be judged in natural light before anything is added.
Which Doje program fits
The Signature Program is Doje's customised plan: it assesses facial structure and ageing pattern, then combines thread lifting, filler, collagen boosters and energy devices only as needed. Where loss is concentrated in one area, the Mid-Face or Eye Program may be the more focused choice.
When a surgical opinion is more appropriate
Fat grafting, a surgical procedure that transfers your own fat, is an alternative for people seeking extensive or longer-lasting volume and requires a qualified surgeon. When marked laxity and excess skin accompany volume loss, a facelift may be more appropriate. Dr. Byun will explain these options openly.
Cost
Fees depend on the areas, products and number of sessions, and are quoted after the 1:1 consultation. Ask for a written quote; Korean non-covered (비급여) aesthetic fees are set per plan.

Frequently asked questions
Why does my face look gaunt after weight loss?
Facial fat compartments shrink along with body fat, and the deep pads that support the cheeks and eyes often show it first.
Which areas should be treated first?
Usually structural points such as the temples, cheeks and chin, because restoring them improves the overall shape and often softens nearby lines.
Fillers or collagen boosters?
Fillers restore defined volume immediately; collagen boosters improve firmness and subtle fullness gradually. Many plans use both.
Will I look overfilled?
Not when volume is restored at key points, staged and combined with lifting where needed. Doje's approach is to use only what your face requires.
How many sessions will I need?
It depends on how much volume has been lost. Larger restorations are often staged over two or more visits.
How long do results last?
It varies by product, area and metabolism. Stable weight helps results last longer.
