Soft Filler vs Structural Filler — Cheongdam & Apgujeong, Seoul

Soft filler is a flexible gel placed in mobile or thin-skinned areas such as under the eyes, lips and fine folds, where it must move naturally. Structural filler is firmer and placed deeper to support ligaments, cheekbones, chin or temples. At Doje Clinic in Cheongdam and Apgujeong, Seoul, Dr. Byun Jeik matches gel type to layer.

Why gel type matters

"Filler" (필러) is not one material. Hyaluronic acid fillers differ in firmness, cohesivity and how much they spread or hold their shape. Those properties determine how a filler behaves once it is inside the face. A firm gel placed under thin skin around the eyes can look lumpy or cast a shadow; a soft gel placed deep on bone to support a ligament may simply spread and provide little lift. The same product in the wrong layer can produce exactly the overfilled or unnatural look patients fear.

So the useful distinction is not brand but job. Soft fillers are chosen to blend and move with expression. Structural fillers are chosen to hold, support and project. Most balanced treatments use both, each in its own layer.

How Doje Clinic uses each

Dr. Byun Jeik has been named one of Korea's 22 filler experts by Medical Times, sits on the Merz Advisory Board and the Belotero Expert Committee, and has demonstrated at the 2024 Juvederm MD Codes event. In practice, his plans are built from the structure outward. Structural filler is placed first, in small amounts, at deep support points — for example retaining-ligament filler in the Mid-Face Program — because restoring support often softens folds without filling them directly. Soft filler is then used only where a residual hollow or line remains, such as marionette soft filler or the Eye Program's soft filler for the upper and lower eye area.

This sequence keeps total volume low, which is central to Doje's aim of natural results.

When Doje combines them

A typical example is a tired mid-face. Structural filler at the ligament support points restores some lift and frames the cheek; soft filler then refines a residual nasolabial or tear-trough shadow. Another example is the jaw and chin: a structural product defines the chin and angle, while a soft product smooths a pre-jowl hollow. If sagging rather than volume loss is the main driver, Dr. Byun may recommend a thread lift first so less filler is needed at all.

Which should you choose?

  • Soft filler — for delicate, thin-skinned or very mobile areas, or to fine-tune a residual line.
  • Structural filler — for deep support, projection or framing where bone or fat has receded.
  • Both — when a face needs support and refinement; this is the most common pattern.
  • A lift instead — when folds come from descent rather than volume; see thread lift vs filler.

Recovery and aftercare

Mild swelling or bruising can occur and usually settles within days. Avoid pressing, massaging or heavy heat on treated areas unless instructed; see filler aftercare.

Cost

Fees are quoted after a 1:1 consultation, since product choice and volume depend on your anatomy. Ask for a written quote; Korean non-covered (비급여) aesthetic fees are set per plan.

Frequently asked questions

What is the difference between soft and structural filler?

Soft filler is a flexible gel for superficial or mobile areas; structural filler is firmer and placed deep for support and projection.

Which is used under the eyes?

Soft filler is usually preferred under the eyes because the skin is thin and the area moves constantly.

Why not use one filler for everything?

Each gel behaves differently in tissue. Using the wrong type for a layer can look lumpy or give little support.

Can filler be dissolved?

Hyaluronic acid fillers can usually be dissolved with an enzyme if needed. This is discussed in consultation.

How long does filler last?

It varies by product, area and metabolism; fillers in deep, static areas often last longer than in mobile areas.
