Key facts
| What it looks like | Wide, heavy mid-face, puffy under-eyes, blunted smile, a face that looks larger than before |
|---|---|
| Main causes | Line-by-line filling, wrong gel for the layer, cumulative top-ups, filling where lifting is needed |
| Prevention | Structure-first planning, small staged amounts, reviews after settling, honest "no" when filler is not the answer |
| If it has already happened | Assessment first; HA filler can often be dissolved, then the face is re-planned |
| Who plans at Doje | Dr. Byun Jeik, 1:1, with a structure-first filler approach |
Why faces end up looking overfilled
Overfilling rarely happens in one appointment. It tends to build up over several, each of which looked reasonable on its own. The common pathways are:
- Chasing lines. Nasolabial folds and under-eye shadows are usually caused by descent and loss of support higher up. Filling the fold itself adds weight where the tissue has already dropped, so the fold returns — and is filled again.
- Using volume to lift. When skin and fat have descended, adding volume can make the face look larger and lower rather than lifted. This is where a thread lift or energy-based lifting is often more appropriate.
- Wrong gel, wrong layer. A soft gel placed deep cannot support; a firm gel placed superficially can show as ridges or puffiness, especially under thin eye skin.
- Top-ups before the old filler has gone. HA filler often persists longer than people expect. Adding more at regular intervals without reassessment can lead to gradual accumulation.
- Treating photos, not faces. Planning from a still, front-on image ignores how the face moves when you smile and talk.
Early warning signs to watch for
- Your cheeks look wider from the front, or your smile pushes up a "shelf" beneath the eyes.
- Under-eye areas look puffy or bluish in some lights, often worse in the morning.
- The lower face feels heavy, or the lip area looks forward-projected in profile.
- People comment that you look "different" rather than "well".
- You notice you need top-ups more and more often to feel the same effect.
None of these signs is a diagnosis — swelling, fluid retention and natural ageing can look similar — but they are worth raising at a consultation.

How Doje Clinic plans to avoid it
Doje's approach to filler grows out of the clinic's wider natural-results philosophy: combine treatments only as far as needed. In practice that means:
- Diagnose the cause. Dr. Byun Jeik assesses bone support, fat pads, ligament strength and skin quality to decide whether the problem is volume, descent, skin or muscle.
- Support before you soften. Small amounts of structural filler at deep support points — for example retaining-ligament filler — are used before any superficial softening.
- Lift when the issue is descent. In the Mid-Face, Marionette and Jawline programs, thread lifting carries the repositioning so filler does not have to.
- Match the gel to the layer. Soft filler for thin, mobile areas; structural filler only where projection and support are needed.
- Stage and review. Results are judged after swelling settles, typically two to four weeks, before anything is added.
Lift or fill? A quick guide
| What you see | More likely cause | Usually considered first |
|---|---|---|
| Deepening smile lines with heavy cheeks | Mid-face descent | Mid-face thread lift, ligament support |
| Hollow, flat cheeks with good skin tone | Volume loss | Structural filler in small amounts |
| Jowls and a blurred jawline | Lower-face descent, fat | Lower-face thread lift, liposuction or ONDA |
| Crepey, thin skin | Skin quality | Skin or collagen boosters |
If you already feel overfilled
Bring any records of previous products and dates if you have them. Where hyaluronic acid filler is the cause, it can often be dissolved with hyaluronidase; the face is then allowed to settle and re-planned, often with less volume and more structural support. Non-HA products need a different, more cautious discussion.
Cost
An honest plan sometimes means less filler, or none. Treatment is quoted after the 1:1 consultation; Korean non-covered (비급여) aesthetic fees are set per plan, and a written quote is available on request.

Frequently asked questions
Why do some people look puffy after filler?
Common reasons are filling lines instead of supporting structure, using the wrong gel for the area, and repeated top-ups before earlier filler has cleared.
Is a little filler always safe from overfilling?
Not always. Overfilling often builds up gradually, so reviewing the face before each top-up matters more than any single amount.
Can overfilled hyaluronic acid filler be corrected?
Often yes. HA filler can usually be dissolved with hyaluronidase, after which the face can be reassessed and re-planned.
When is lifting better than filler?
When tissue has descended, lifting with threads or an energy device usually repositions it more naturally than adding volume.
How long should I wait before judging a filler result?
Typically two to four weeks, once swelling has settled.
Does Doje Clinic ever recommend no filler?
Yes. If filler is not the right tool for your face, Dr. Byun Jeik will say so and suggest an alternative.
