Sagging Skin After Weight Loss — Cheongdam & Apgujeong, Seoul

Sagging skin after weight loss happens when fat volume shrinks faster than the skin and its support ligaments can recoil. At Doje Clinic in Cheongdam and Apgujeong, Seoul, Dr. Byun Jeik separates lost volume from loose skin and descent, then combines filler, thread lifting or energy devices only as needed; large excess skin needs a surgeon.

Why skin sags after weight loss

Weight loss changes the face and body from the inside out. Fat is not one uniform layer: in the face it sits in separate compartments — around the cheeks, under the eyes, at the temples and along the jaw — each held in place by fibrous retaining ligaments. When those compartments shrink, the skin that was stretched over them does not always contract to match. The result is a mixture of three different problems that often appear together:

  • Deflation — hollow temples, flattened cheeks and deeper under-eye grooves where fat volume used to be.
  • Laxity — skin that has lost some of its elastic recoil after being stretched, especially after large or repeated weight changes.
  • Descent — soft tissue sliding downward because the volume that once supported it has gone, which deepens nasolabial folds and creates jowls.

How much each factor contributes depends on age, how quickly the weight came off, how long the skin was stretched, genetics, sun exposure and smoking history. Younger skin with healthy collagen often rebounds well; skin in the 40s and beyond, or after very rapid loss — including weight loss supported by appetite-suppressing medication — more often shows a gaunt or tired look. This is why two people who lose the same weight can look very different afterwards.

How Doje Clinic assesses post-weight-loss laxity

Doje’s philosophy is that care should change as the face changes, and post-weight-loss skin is a clear example. In a 1:1 consultation, Dr. Byun Jeik, a Seoul National University–trained surgery specialist, looks at the face at rest and in motion to work out which of the three problems dominates. A face that is mainly deflated needs volume in the right compartments; a face that is mainly lax needs collagen stimulation and tightening; a face that has descended needs repositioning. Treating all three the same way is how faces end up overfilled or still sagging.

Where useful, Mark-View 3D skin analysis adds objective data on skin texture and condition. Dr. Byun will also ask whether your weight is stable, whether you are still losing, and whether you plan further change — because volume placed into a face that keeps slimming can look very different a few months later.

Two faces, same treatment, different results.

The difference is the design. Tell us your goals and dates; in your 1:1 consultation Dr. Byun Jeik shows you what your face needs, and what it doesn't.

Options from mild to advanced

  1. Skin quality first (mild laxity, dull or thin skin). Collagen and skin boosters such as GOURI, Re2O or Rejuran aim to improve thickness, hydration and texture gradually over several weeks.
  2. Energy-based tightening (mild to moderate laxity). Ultherapy Prime, Thermage or Softwave heat targeted layers to stimulate collagen remodelling. Doje selects a device by depth, degree of sagging, fat and elasticity rather than applying every device as a package.
  3. Volume restoration (deflation). Soft or structural filler restores specific compartments; Radiesse, a calcium hydroxylapatite filler, adds support while stimulating collagen.
  4. Repositioning (descent). A Doje Signature Thread Lift(실리프팅) lifts descended tissue and is designed around face shape and sagging pattern.
  5. Combination plans. Most post-weight-loss faces need two or three of these layers in a planned sequence, not everything at once.

Where Doje’s programs fit

For the face, the Signature Program is the usual starting point because it is built around individual structure and ageing pattern. If the change is concentrated in one zone, a regional program may suit better: the Mid-Face Program for flattened cheeks and smile lines, or the Jawline Program when the lower face has lost definition. For the body, Doje’s Body Program focuses on contour and volume (body filler, body injections, Body ONDA) — it is not a substitute for removing excess skin.

When a surgeon is the right call

Non-surgical treatment works on quality, volume and moderate lift. It cannot remove skin. If you can pinch and lift a fold of loose skin on the neck, abdomen, upper arms or thighs, or if facial skin hangs in heavy folds, a consultation with a plastic surgeon about skin-excision procedures is more appropriate. Doje does not perform those operations and will tell you honestly when your goal is beyond what injectables, threads and devices can achieve.

What to expect

  1. 1:1 consultation with Dr. Byun: history of weight change, goals and timeline.
  2. Assessment of volume, laxity and descent; Mark-View 3D where helpful.
  3. A written plan prioritising the most visible problem first.
  4. Treatment, often staged over weeks so each step can settle.
  5. Review and maintenance as collagen remodelling continues.

Cost

Plans are quoted after a 1:1 consultation because the combination and sequence vary widely between patients. Aesthetic treatment in Korea is non-covered (비급여), and fees are set per plan — ask for a written quote before you commit.

Frequently asked questions

Should I wait until my weight is stable before treatment?

Usually yes. Dr. Byun generally prefers your weight to be stable for a few months so that volume and lift are planned for the face you will keep.

Can filler fix a gaunt face after weight loss?

Filler can restore lost volume in specific compartments, but if the main issue is loose skin or descent, filler alone can look heavy. It is often combined with lifting or tightening.

Do energy devices tighten loose skin after weight loss?

They can improve mild to moderate laxity by stimulating collagen, with results developing over months. They do not remove excess skin.

Can Doje treat loose skin on the body?

Doje’s Body Program addresses contour and volume. Large folds of excess body skin are better assessed by a plastic surgeon.

Is facial change from weight-loss medication treated differently?

The same principles apply: identify deflation, laxity and descent, then plan accordingly. Speed of loss is discussed because rapid change often affects volume more.

How much does treatment cost?

Costs are quoted after a 1:1 consultation because plans differ. Korean aesthetic fees are non-covered and set per plan, so ask for a written quote.
