What causes melasma?

Melasma (기미) is not simply a cluster of sun spots. It is a chronic condition in which pigment cells (melanocytes) become over-active and over-responsive. Several triggers interact:

  • Light. Ultraviolet light is the main driver, but visible light and infrared heat also stimulate pigment in melasma-prone skin. This is why melasma often darkens even with sunscreen, and after saunas or hot kitchens.
  • Hormones. Melasma is far more common in women and often begins or worsens during pregnancy or with hormonal contraception.
  • Genetics and skin type. It is more common in medium skin tones, including many East and Southeast Asian patients, and often runs in families.
  • Inflammation and the dermis. Research suggests that changes in the dermis, small blood vessels and the skin barrier help sustain the pigment, which helps explain why melasma relapses.

Pigment can sit in the epidermis, the dermis, or both (mixed). Deeper pigment usually responds more slowly. Melasma also commonly coexists with freckles, sun spots and post-inflammatory pigmentation, which look similar but behave differently.

How Doje Clinic approaches melasma

Because melasma is easily aggravated, Doje’s approach is deliberately measured. In a 1:1 consultation, Dr. Byun Jeik examines the pattern and distribution of pigment and uses Mark-View 3D skin analysis to record pigmentation data for planning and later comparison. He will ask about pregnancy plans, hormonal medication, sun and heat exposure, and any previous laser treatment that made things worse.

Doje uses Helios 785/1064 dual toning as the base for pigmentation, adjusting intensity and the number of sessions to the depth and pattern of pigment rather than pushing settings for quick change. Treating melasma too aggressively can trigger rebound darkening or new pigmentation, so the aim is steady, sustainable improvement.

Two faces, same treatment, different results.

The difference is the design. Tell us your goals and dates; in your 1:1 consultation Dr. Byun Jeik shows you what your face needs, and what it doesn't.

Options from mild to advanced

  1. Daily protection. Broad-spectrum sunscreen reapplied through the day, ideally tinted to help with visible light, plus hats and shade. This is the foundation of every plan.
  2. Calm the skin. Stopping irritating products and supporting the barrier with Doje’s soothing and hydration care reduces inflammation that can feed pigment.
  3. Helios dual toning. A series of low-intensity sessions spaced over weeks, with settings tuned to your pigment. Mild redness usually settles quickly.
  4. Skin quality support. Where the barrier is weak, skin boosters may be added selectively to support overall skin condition.
  5. Medical therapy. Prescription creams or oral medication for melasma are managed by a dermatologist or your own doctor and can be used alongside in-clinic care.

Where Doje’s programs fit

Melasma care sits in the pigmentation line of Doje’s Skin & Energy menu. If you are also planning lifting, Dr. Byun considers how energy devices and heat might affect your pigment and sequences treatments accordingly. For events, a pre-event timeline helps, since toning works gradually.

When a dermatologist is more appropriate

See a dermatologist if the diagnosis is uncertain, if a single patch is changing in shape, colour or texture, if pigment appeared after starting a new medication, or if you want prescription treatment. During pregnancy and breastfeeding, most laser and prescription options are postponed; discuss safe skincare with your doctor.

Frequently asked questions

Can melasma be cured?

Melasma is chronic and can relapse, so the aim is control and gradual lightening. Daily sun protection and maintenance sessions are usually needed.

Why does melasma come back in summer?

Ultraviolet light, visible light and heat all stimulate pigment in melasma-prone skin. Protection matters year-round, especially in summer.

Is Helios toning safe for melasma?

Toning is used at intensities adjusted to pigment depth and pattern. Dr. Byun avoids aggressive settings, which can worsen melasma.

How many toning sessions will I need?

Melasma usually needs a series of sessions over weeks. The number depends on pigment depth and response.

Can I treat melasma during pregnancy?

Most laser and prescription treatments are postponed during pregnancy and breastfeeding. Gentle skincare and sun protection are the focus.

How much does melasma treatment cost?

It is quoted after a 1:1 consultation. Korean aesthetic fees are non-covered and set per plan, so request a written quote.
