Why the face gets heavier at the bottom
A youthful face tends to carry its fullness high, around the cheekbones, and narrows towards the chin. With time that shape often reverses: the upper and mid-face look flatter while the area around the mouth and jaw looks fuller and heavier. In Korea this is commonly described as a heavy 하관 (lower face). It is a proportion problem rather than a single feature, and it rarely has a single cause.
- Deflation above. Deep fat pads in the mid-face thin and the bony support of the cheek recedes, so the upper face loses fullness and the lower face looks heavier by comparison.
- Descent from above. As retaining ligaments loosen, cheek fat migrates down towards the mouth and jaw, literally moving volume from the top of the face to the bottom.
- Extra fat below. Some people carry more subcutaneous fat in the lower cheeks, jowls and under the chin, which becomes more prominent as the skin loosens.
- Muscle bulk. A large masseter muscle widens the jaw angle and adds width low on the face.
- Skin laxity. Looser skin lets every one of these changes show more.
How Doje Clinic assesses facial balance
Dr. Byun Jeik looks at the whole face before looking at the lower third. In a 1:1 consultation he compares the proportions of the upper, middle and lower face, then asks three questions of each area: has it descended, is there too much, or is there too little? The answers often surprise people — a heavy lower face may need support in the mid-face as much as reduction at the jaw.
- Clench test and palpation to tell muscle bulk from fat.
- Pinch and recoil to judge fat thickness and skin elasticity.
- Profile and three-quarter views to see where descent has occurred.
- Mark-View 3D skin analysis for objective skin data where skin quality affects the plan.
This is the clinic's broader principle in action: care is combined only as far as needed, rather than every option being applied to every face.

Options from mild to advanced
Reduce where there is excess
Where fat is the issue, ONDA can reduce subcutaneous fat gradually, and double-chin liposuction addresses a defined pocket under the chin. Where a wide jaw muscle is responsible, jaw (masseter) Botox can slim the angle over several weeks.
Lift what has descended
When volume has migrated downward, the Doje signature lower-face thread lift moves it back towards where it came from. Energy lifting such as Ultherapy Prime or Softwave can support the deeper layer and the skin. The clinic states 17 years of accumulated thread-lift experience and thread results that can last up to two years or more; individual results vary.
Support what has deflated
Restoring a little structure in the mid-face — for example with retaining-ligament filler as part of the Mid-Face Program — shifts visual weight upward and can make the lower face look lighter without touching it. Restraint matters: the aim is balance, not a fuller face overall.
Sequence matters
Reducing volume from a lax lower face without lifting can make sagging more obvious, while adding volume above without lifting below can make the whole face look heavier. Doje plans the order so that each step supports the next, sometimes over more than one visit.
Which Doje program fits
For most people the Jawline Program — lower-face thread lift and double-chin liposuction, with Radiesse, Ultherapy Prime and ONDA as options — is the starting point. When mid-face support is also needed, the Signature Program lets Dr. Byun combine lifting, filler, collagen boosters and energy devices across the face in a single customised plan.
When a surgical opinion is more appropriate
If heavy jowls and loose neck skin dominate, a lower facelift assessed by a qualified surgeon may give a more proportionate result than layering non-surgical treatments. Some people also ask about surgical removal of cheek fat; this permanently removes volume that the face may miss with age and needs careful surgical assessment. Doje's own approach is non-surgical and conservative.
Cost
Because a rebalancing plan can involve one area or several, fees are quoted after the 1:1 consultation. Ask for a written quote; Korean non-covered (비급여) aesthetic fees are set per plan.

Frequently asked questions
Why does my face look heavier at the bottom as I age?
Volume tends to move downward: the mid-face deflates while fat and skin descend towards the mouth and jaw. The lower face looks heavier even if its own volume has not changed much.
Will slimming the lower face make me look older?
It can if volume is removed from a face that is already lax. That is why Doje pairs reduction with lifting or support when the skin needs it.
Should I lift or reduce first?
It depends on the cause. Fat-dominant faces often start with reduction; descent-dominant faces with lifting. The order is set in consultation.
Can jaw Botox help a heavy lower face?
When a large masseter muscle adds width, yes, gradually over several weeks. It does not lift sagging tissue or reduce fat.
Can everything be done in one visit?
Some combinations can be done the same day; others are staged so each step can settle. Dr. Byun will advise based on your plan and travel dates.
What is the downtime?
It varies by method, from little or none for toxin and most devices to several days for threads and longer for liposuction.
