Why layer-by-layer thinking matters
Most disappointing aesthetic results share one cause: the right treatment in the wrong layer. Filling a fold whose real cause is a sagging cheek, or using a skin laser to try to lift a jowl, rarely gives the result people hope for. Thinking in layers turns a vague concern such as “I look tired” into a specific question: which layer has changed, and by how much?
The layers, from deep to surface
- Bone: the facial skeleton slowly remodels. The eye sockets tend to widen, the upper jaw loses forward projection and the jawline can lose definition. Less scaffold means the soft tissue has less to drape over.
- Retaining ligaments and SMAS: the anchors and supportive sheet loosen, so tissue between anchor points begins to slide. This drives sagging cheeks, jowls and deeper folds.
- Fat compartments: the face has separate superficial and deep fat pads. Deep pads that prop up the cheek and eye area tend to deflate, while superficial fat descends. That is why a face can look hollow and heavy at the same time.
- Muscle: years of expression etch frown, forehead and crow's-feet lines. Some muscles, such as those pulling the mouth corners and the platysma in the neck, can have a more noticeable downward effect over time.
- Skin: collagen and elastin decline, hydration falls, renewal slows, and sun exposure adds pigment and texture changes. Skin also becomes thinner, revealing what lies beneath more clearly.

How the surface layer is measured
Mark-View 3D skin analysis provides objective data on surface features such as pores, pigment and texture, which helps track change over time. Deeper layers are assessed by examination: the doctor looks at you at rest and in expression, from several angles, and feels tissue quality and laxity.
How Doje uses this
Doje Clinic's approach is to assess facial structure, ageing pattern and goals, then combine thread lifting, filler, collagen boosters and energy devices only as far as needed. Energy devices are chosen by depth, sagging, fat and elasticity rather than applied as a package. In practice, most people have one or two dominant layers, and treating those well usually matters far more than treating everything a little. The Signature program brings this together in a single bespoke plan. See the Signature program.

Frequently asked questions
Which layer ages first?
Skin and muscle changes are often noticed first, while deeper changes in fat, ligaments and bone become visible later. In reality all layers change gradually together.
Does facial bone really change with age?
Yes. Gradual remodelling around the eye sockets, mid-face and jaw is well described and reduces the support beneath the soft tissue.
Can one treatment fix every layer?
No. Each treatment works at particular depths, which is why plans often combine a small number of well-chosen steps.
Why does my face look hollow and heavy at once?
Deep fat pads deflate, causing hollows, while superficial fat descends, adding heaviness lower down.
How do I know which layer is my main issue?
A 1:1 examination at rest and in expression, sometimes with Mark-View 3D for the skin, identifies the dominant layer.
