What actually forms a jowl
Along the jawline, the soft tissue is held by a row of retaining ligaments. The mandibular ligament anchors skin to bone just in front of the jowl, while the masseteric ligaments support the tissue over the chewing muscle further back. Between them sits a compartment of fat. In youth this fat stays above the jaw border; with time the masseteric support weakens, the fat and overlying skin slide down, and the firmly anchored mandibular ligament stays put. The result is the classic pattern: a bulge over the jaw with a small dip — the pre-jowl sulcus — in front of it.
Several other changes add to the picture:
- SMAS and skin laxity. The fibromuscular SMAS layer and the skin above it lose elasticity, so they no longer hold the tissue up against gravity.
- Bone change. The jaw gradually remodels with age, especially in front of the jowl, which deepens the sulcus and exaggerates the bulge behind it.
- Fat volume. Some people simply carry more fat in the lower face; others jowl early with very little fat because laxity dominates.
- Neck pull. The platysma muscle in the neck pulls downwards on the lower face, and its effect becomes more visible as support weakens.
Three types of jowl — and why it matters
At Doje, the first job is to decide which of three patterns is driving your jawline change, because each responds to different tools:
- Laxity-dominant: modest fat, loose support. Lifting is the priority.
- Fat-dominant: fuller lower face with good skin. Reducing fat may do more than lifting.
- Hollow-dominant: the pre-jowl dip is the main problem. Restoring jawline continuity can make the jowl look smaller without lifting it.
Most people show a mix. Dr. Byun Jeik assesses this in a 1:1 consultation, often alongside Mark-View 3D skin analysis, and looks at the jaw in profile and with the head tilted down, where jowls are most honest.

Options from mild to advanced
Early jowling: tighten the support layer
When the jawline is only softening, energy devices may be enough. Ultherapy Prime delivers focused ultrasound to the deeper support layer, while Softwave offers a gentler, broader heating pattern. Changes build gradually over the following months and are typically modest but natural.
Fullness along the jaw: address the fat
If extra subcutaneous fat is the main issue, ONDA, a microwave-based device, can be used to target fat and contour. Where fat extends under the chin, double-chin liposuction may form part of the plan. Lifting a heavy jowl without addressing the fat often gives a short-lived result.
Clear descent: lift with threads
When tissue has visibly moved below the jaw, the Doje signature lower-face thread lift(턱라인 실리프팅) repositions it along vectors chosen for your face shape. The aim is an immediate, natural line without lumpiness. The clinic states 17 years of accumulated thread-lift experience and that results can last up to two years or more; individual results vary.
The pre-jowl dip: restore continuity
A small, precisely placed amount of support filler or Radiesse in front of the jowl can smooth the transition from chin to jaw. This is about continuity, not adding bulk to the lower face.
Which Doje program fits
The Jawline Program is built for jowls and blurred jawlines: lower-face thread lift and double-chin liposuction, with Radiesse, Ultherapy Prime and ONDA as options chosen by structural sagging, subcutaneous fat and skin elasticity. If jowls come with marionette lines or a downturned mouth, the Marionette Program may be the better framework. Doje uses only the elements your face needs, not a fixed package.
When a surgical opinion is more appropriate
If loose skin extends from the jaw into the neck, if there are prominent neck bands with heavy skin, or if you want a single, larger change, a lower facelift or neck lift assessed by a qualified surgeon may be more suitable than threads and devices. Dr. Byun will tell you honestly when non-surgical options are unlikely to meet your expectations; see thread lift vs surgical facelift.
Cost
Your plan, and therefore your fee, is set after the consultation because jowl treatment can range from a single device session to a combined lifting and contouring plan. Ask for a written quote; Korean non-covered (비급여) aesthetic fees are set per plan.

Frequently asked questions
Can jowls be treated without surgery?
Mild to moderate jowls often respond well to a combination of thread lifting, energy lifting and contouring. Heavy laxity extending into the neck usually needs a surgical opinion.
Does jaw Botox help jowls?
Not directly. Masseter toxin slims a wide jaw muscle but does not lift descended tissue; in a laxity-dominant face it can even make sagging more noticeable, so it is used selectively.
Can ONDA remove jowl fat?
ONDA uses microwave energy aimed at subcutaneous fat and may help fat-dominant jowls. It is not a substitute for lifting when laxity is the main cause.
Why do my jowls look worse in photos looking down?
Tilting the head lets gravity pull the loosened tissue forward over the jaw, so it is the angle that reveals laxity most clearly and is part of the assessment.
How long does a lower-face thread lift last?
The clinic states its thread-lift results can last up to two years or more, but duration varies with skin quality, fat, age and lifestyle.
What is the downtime?
Energy devices typically have little visible downtime. After threads, mild swelling and tightness for several days is common; after liposuction, a longer recovery with compression is usual.
