Why the area under the eyes sinks

The lower eyelid sits on a delicate stack of layers: very thin skin, the ring-shaped orbicularis muscle that closes the eye, a pad of fat beneath that muscle, and the bony rim of the eye socket. In youth, the lid flows smoothly into the cheek. Hollowness — 눈밑 꺼짐 in Korean — develops when that continuous surface loses its support.

• Soft-tissue thinning. The fat that cushions the area below the eye and over the upper cheek reduces with age, so the skin sits closer to the bone.
• A wider eye socket. The bony rim of the orbit gradually remodels and widens, especially along its lower edge, leaving the tissue above it with less scaffolding.
• Thin skin. Lower-lid skin is among the thinnest on the body. As collagen declines, it reveals every contour and shadow beneath.
• Genetics and build. Some people inherit a deeper-set eye or a flatter mid-face and notice hollows in their twenties. Weight loss and intense training can also reveal them.

Under-eye hollows are related to, but not the same as, a tear trough: the trough is a specific groove near the inner corner, whereas hollowness can extend broadly across the lid and the upper cheek.

The eye area is one where a small error is easy to see, so assessment is deliberately careful. In a 1:1 consultation, Dr. Byun Jeik — named one of Korea's 22 filler experts by Medical Times — examines the area under different lighting, at rest and when you look upward, and checks:

• whether hollowness is accompanied by puffiness or fat bags, which changes what filler can do;
• skin thickness and colour, which affect how visible any product would be;
• whether the cheek below has flattened or descended, making the eye look hollow from beneath;
• any history of persistent swelling, allergies or previous filler in the area.

Options from mild to advanced

Thin, crepey skin with mild hollowing

When the issue is mainly skin, eye-area skin boosters come first. Eye Rejuran and Re2O Fine are designed for the delicate lower lid and aim to improve thickness and resilience over a series of sessions. Eye Thermage with Softwave may be used to tighten the skin. Improvement is gradual.

Moderate hollows

Under-eye soft filler restores the lost support. Doje uses a soft filler suited to delicate areas and places it conservatively, usually deep against the bone, so that it supports rather than sits under the thin skin. Too much product, or product placed too superficially, can cause puffiness or a bluish tint (the Tyndall effect), which is why small amounts and careful technique matter more than speed.

Hollows that start in the cheek

If the cheek beneath the eye has flattened or descended, the eye looks hollower than it is. Supporting the mid-face — with retaining-ligament filler or the mid-face thread lift from the Mid-Face Program — can reduce the need for filler directly under the eye.

Combined approach

Many people benefit from both volume and skin work: a modest amount of soft filler to restore contour, followed by skin boosters to improve the surface over it.

Which Doje program fits

Frequently asked questions

Why do I have under-eye hollows in my twenties?

Genetics, a deep-set eye shape, a flatter mid-face or low body fat can all cause early hollows. They are not necessarily a sign of ageing.

Is under-eye filler safe?

In experienced hands with a suitable soft filler and careful technique, it is a widely performed treatment, but like any injection it carries risks such as bruising, swelling and, rarely, vascular complications. These are discussed in consultation.

What is the Tyndall effect?

A bluish tint that can appear when filler is placed too close to thin skin. Deep placement and conservative amounts reduce the risk.

How long does under-eye filler last?

The eye area moves little, so filler often lasts relatively long there, but duration varies with the product and the individual.

Can skin boosters fix hollows?

They improve skin thickness and quality, which softens shadowing, but they do not replace lost volume in deeper hollows.

What is the downtime?

Mild swelling and possible bruising for several days are common after filler; skin boosters may cause small bumps that settle within a few days.
