Ultherapy vs Thermage — Cheongdam & Apgujeong, Seoul

Ultherapy uses focused ultrasound to reach deeper support layers such as the SMAS, so it suits sagging jowls and lower-face heaviness. Thermage uses radiofrequency to tighten the skin itself, so it suits crepey, thinning skin. At Doje Clinic in Cheongdam and Apgujeong, Seoul, Dr. Byun Jeik often chooses by depth, or combines them.

Key facts

Core difference Ultherapy Prime (울쎄라) delivers micro-focused ultrasound to precise depths, including the SMAS; Thermage FLX (써마지) delivers monopolar radiofrequency that warms the dermis in a broad, even layer.
Ultherapy typically suits Structural sagging — jowls, a heavy lower face, a soft jawline, mild brow descent.
Thermage typically suits Skin-quality laxity — crepey or thinning skin, loss of surface firmness, eyelid skin (Eye Thermage).
Downtime Usually minimal for both; temporary redness, swelling or tenderness varies.
Results timeline Some early tightening; collagen remodelling typically builds over 2–3 months.

The short answer

Ultherapy and Thermage are the two names international patients ask about most when they want lifting without surgery in Seoul, and they are often described as if they were interchangeable. They are not. The real question is which layer of your face is actually causing the change you see in the mirror. If the problem is that tissue has descended — the cheek fat pad sliding toward the nasolabial fold, jowls breaking the jawline — the support layer needs attention, and that is Ultherapy's territory. If the problem is that the skin itself has become thinner, looser and finely creased, the dermis needs attention, and that is where Thermage tends to shine.

Many faces in their 40s and 50s show both patterns at once, which is why the answer at Doje Clinic is rarely just one device by default.

How each device works

Ultherapy Prime: focused points at set depths

Micro-focused ultrasound converges energy into tiny thermal coagulation points beneath an untouched skin surface. Different transducers place these points at different depths — deep enough to reach the SMAS (the fibromuscular layer that surgeons lift in a facelift) or shallower in the dermis. The Prime generation adds real-time ultrasound imaging, so the operator can see the tissue layers before firing. Dr. Byun Jeik was the Ultherapy Transducers winner at the 2024 Merz Golden Record Award, and transducer selection by depth is central to how he plans it.

Thermage FLX: volumetric heating of the skin

Thermage uses monopolar radiofrequency, which heats a wide volume of dermis and the fibrous septae beneath it rather than discrete points. Collagen contracts with the heat and new collagen is stimulated over the following months. Because the energy is spread evenly across the surface, Thermage is well suited to improving skin firmness and texture over large areas, and a dedicated eye tip allows treatment of upper and lower eyelid skin.

When Doje combines them

Doje Clinic explicitly avoids applying every device as a package. A combination is considered only when assessment shows two distinct problems — for example, mid-face descent and thinning, crepey skin over the cheeks. In that case Ultherapy may address the deeper support while Thermage targets the dermis, sometimes in the same visit, with energy levels adjusted so the face is not over-treated. Where skin is thin or fat volume is already low, Dr. Byun may reduce or skip one device, because excessive heating of a lean face can make it look gaunt.

For patients whose descent is more advanced, a thread lift (실리프팅) may give the mechanical repositioning that neither device can, with energy treatment used as support. These decisions are made within the Mid-Face or Jawline programmes rather than device by device.

Which should you choose?

  • Choose Ultherapy if your main concern is heaviness or sagging along the jawline, jowls or under the chin, and your skin is still reasonably thick.
  • Choose Thermage if the skin looks crepey, finely lined or less springy, but the overall contour is still in place.
  • Consider both if you have structural descent and poor skin quality together — but only after an assessment confirms both.
  • Consider neither alone if sagging is marked; threads or other options may be more effective.
  • Eyelid skin is a specific case where Eye Thermage is usually the relevant option.

Recovery and aftercare

Most people return to normal activities the same day. Gentle skincare, sun protection and avoiding saunas and intense heat for a few days are typical advice; see aftercare for energy devices.

Cost

Fees are quoted after a 1:1 consultation because the device, the number of lines or shots and any combination depend on your assessment. Ask for a written quote; Korean non-covered (비급여) aesthetic fees are set per plan.

Frequently asked questions

Is Ultherapy or Thermage better for jowls?

Ultherapy is usually the more relevant choice for jowls because it can reach the SMAS support layer. Thermage may help if the skin over the jowls is also lax.

Can I have Ultherapy and Thermage on the same day?

Sometimes, yes, when both deep sagging and skin-quality laxity are present. Energy levels are adjusted so the face is not over-treated.

Which hurts more?

Sensations differ: Ultherapy tends to feel deep and achy over bony areas, while Thermage feels like brief heat pulses. Comfort measures are discussed beforehand.

Will either make my face look thinner?

In lean faces, aggressive heating can reduce soft volume, which is why Doje adjusts depth and energy to fat and elasticity rather than using fixed settings.

How soon will I see results?

Some tightening may be noticed early, but collagen remodelling typically develops over two to three months, and results vary.
