The SMAS in plain English
Imagine the face as a carefully layered structure rather than a single sheet of skin. Just under the skin is a cushion of fat. Beneath that lies the SMAS: a thin but strong layer of fibrous tissue interwoven with muscle in places. It behaves a little like the supportive lining of a well-tailored jacket, holding everything above it in shape and transmitting the pull of the expression muscles so that the face moves as a unit when you smile or speak.
The SMAS is not identical across the face. Over the cheek it is a defined fibrous layer; lower down it blends into the platysma, the thin muscle sheet of the neck; above the cheekbone it continues as the fascia over the temple. Retaining ligaments pass through it, anchoring the skin and SMAS to the bone beneath at specific points.
Where it sits among the five layers
Anatomists often describe the face in five broad layers, from the surface inwards:
- Skin: epidermis and dermis, where texture, pigment and fine lines live.
- Subcutaneous fat: divided into compartments separated by fibrous walls.
- SMAS and the muscles of expression: the supportive, mobile layer.
- Retaining ligaments and soft-tissue spaces: the anchoring and gliding layer that lets the face move.
- Deep fascia and periosteum: the lining over the facial bones, with deep fat pads in some areas.

What happens to the SMAS with age
Over time the SMAS stretches and loosens, and the retaining ligaments that tether it become more lax. Gravity, repeated movement and the gradual loss of deep fat and bone support beneath all add to the effect. The result is that the SMAS and the fat it carries slide downwards and forwards. This is a major reason why jowls form along the jawline, why the cheek looks flatter and lower, and why the nasolabial fold deepens. It also explains why treatments that only work on the skin surface cannot, by themselves, lift a sagging face.
How treatments relate to the SMAS
| Treatment | Layer mainly reached | Typical effect |
|---|---|---|
| Surgical facelift | SMAS repositioned and secured | The most structural option; requires surgery |
| Ultherapy Prime | Micro-focused ultrasound can reach around SMAS depth | Gradual tightening and collagen stimulation |
| Thermage | Dermis and fibrous tissue beneath | Skin tightening and firmness |
| Thread lift | Usually placed in the fat layer above the SMAS | Repositions soft tissue; immediate lift |
| Retaining-ligament filler | Deep support points | Restores support so tissue sits higher |
Non-surgical treatments do not reproduce the repositioning achieved in surgery, but used at the right depth they can tighten, support and reposition tissue in a natural-looking way.
Why depth matters at Doje
Doje Clinic chooses energy devices by depth, sagging, fat and elasticity, and explicitly avoids applying every device as a package. Understanding whether your laxity lies mainly in the SMAS, the fat or the skin is what makes that choice meaningful. In a 1:1 consultation, Dr. Byun Jeik, a surgery specialist trained at Seoul National University Hospital, assesses each layer before recommending a plan. See energy devices and treatment depth.
Cost
Any treatment plan is quoted after a 1:1 consultation, with a written quote on request. Korean non-covered (비급여) aesthetic fees are set per plan.

Frequently asked questions
What does SMAS stand for?
Superficial musculoaponeurotic system: a fibromuscular layer beneath the skin and fat that connects the facial muscles of expression.
Can non-surgical treatments tighten the SMAS?
Micro-focused ultrasound such as Ultherapy Prime is designed to deliver energy at around SMAS depth, which can tighten tissue gradually. It does not reposition the SMAS as surgery does.
Does a thread lift lift the SMAS?
Threads are usually placed in the fat layer above it and reposition the tissue they hold. They work alongside, rather than replace, deeper support.
Is the SMAS a muscle?
Not exactly. It is a fibrous layer that contains or connects to muscle in places, such as the platysma in the neck.
Why do jowls form?
Mainly because the SMAS and fat loosen and slide forward and down past the firm anchor of the mandibular ligament near the chin.
