What causes under-eye bags
Behind the lower eyelid, cushions of fat surround and protect the eyeball. A thin membrane called the orbital septum, together with the orbicularis muscle in front of it, normally holds this fat in place. With age — and in some families, from early adulthood — the septum and muscle loosen and the fat bulges forward. This is the classic fat bag, called 눈밑 지방 in Korean.
Bags can also come from other sources, and the distinction is important:
- Fat bags. Present all day, more prominent when you look upward, and slowly progressive over years.
- Fluid puffiness. Worse in the morning, after salty food, alcohol, poor sleep or allergies, and better later in the day.
- Skin and muscle laxity. Loose, crepey skin that folds when you smile, rather than a smooth bulge.
- Malar mounds or festoons. Swellings lower down, on the upper cheek, often fluid-related and notoriously difficult to treat.
Persistent puffiness that appears suddenly, affects one side or comes with other symptoms deserves medical review, as thyroid, kidney, allergic or other conditions can cause swelling around the eyes.
An honest assessment at Doje Clinic
Many people hope an injection or device can make true fat bags disappear. In a 1:1 consultation, Dr. Byun Jeik — a surgery specialist trained at Seoul National University Hospital — will tell you plainly whether that is realistic. He examines the eye with your gaze straight ahead and upward, checks how the bulge changes through the day if you have noticed variation, and looks at the groove beneath the bag and the condition of the skin.
- If fat prolapse is the main cause, non-surgical care can only refine the edges.
- If a groove beneath a mild bag exaggerates it, smoothing that transition may make the bag much less noticeable.
- If the skin is the main issue, skin tightening and boosters are appropriate.
- If fluid dominates, filler is usually avoided because it can make puffiness worse.
This candour is part of the clinic's approach: combining treatments only as far as needed, and not using tools for problems they cannot solve.

Options from mild to advanced
Fluid-type puffiness
Lifestyle measures — sleeping with the head slightly raised, reducing salt and alcohol, treating allergies — often help more than any procedure. Supportive care such as oxygen therapy may make the area feel fresher, but filler is generally avoided in fluid-prone eyes.
Mild bags with a groove beneath
When a small bag sits above a tear trough, the shadow below makes it look larger. A very conservative amount of under-eye soft filler placed below the bag can blend the lid into the cheek. This camouflages rather than removes the bag and is suitable only for selected cases.
Lax, crepey skin over the bag
Where the skin itself is loose, Eye Thermage combined with Softwave can tighten the lower lid gradually, and skin boosters such as Eye Rejuran or Re2O Fine improve thickness and texture over a series of sessions.
Supporting the cheek
If the cheek below has descended, the lid-cheek junction lengthens and bags look more pronounced. Mid-face support can shorten that junction and make the area look smoother.
Which Doje program fits
Within the Eye Program — lifting with the Doje signature upper-face thread lift, volume with soft filler and eye-area skin improvement, with Eye Thermage with Softwave, GOURI/Re2O, Eye Rejuran and Re2O Fine as options — Doje addresses the contour and skin around bags. Doje's options here are non-surgical and do not remove fat.
When a surgical opinion is more appropriate
If prominent fat is the main cause, lower-eyelid surgery (blepharoplasty) is the treatment that removes or repositions it. Depending on skin quality, a surgeon may operate through the inside of the eyelid or through a fine incision below the lashes, and fat can often be moved into the groove beneath. This needs assessment by a qualified oculoplastic or aesthetic surgeon. Dr. Byun will recommend this openly rather than add filler that could make the area heavier.
Cost
Where non-surgical care is appropriate, fees depend on the combination of contour and skin treatments and are quoted after the 1:1 consultation. Ask for a written quote; Korean non-covered (비급여) aesthetic fees are set per plan.

Frequently asked questions
Can under-eye bags be removed without surgery?
True fat bags cannot be removed without surgery. Non-surgical care can soften the groove around mild bags and improve the skin, which can make them less noticeable.
Will filler make my bags worse?
It can if the bags are fluid-related or if filler is placed into or beside a significant bulge. That is why Doje uses filler only in selected cases and in small amounts.
Why do I have eye bags in my twenties?
Prominent lower-lid fat is often inherited and can appear early. In that case it is a structural feature rather than a sign of ageing or tiredness.
Can Eye Thermage help bags?
It can tighten lax lower-lid skin, which helps when skin laxity contributes, but it does not remove protruding fat.
Why are my eyes puffier in the morning?
Fluid collects around the eyes when lying flat, especially after salt, alcohol or poor sleep. This type of puffiness usually improves during the day.
When should I see a surgeon?
When bags are present all day, worsen on looking up and are your main concern, a surgical assessment for lower-eyelid surgery is the logical next step.
