Movement versus volume
Botox and filler are both injections and are often mentioned in the same breath, but they solve opposite problems. Botox works on movement: it reduces the activity of specific muscles, so the lines they fold into the skin soften. Filler works on volume and structure: it replaces what has been lost or supports what has descended, so shadows and hollows soften.
A simple mirror test helps. Frown, raise your eyebrows and smile widely, then relax. Lines that appear only during expression and disappear at rest are dynamic — Botox territory. Lines, folds and hollows that remain with your face completely still are static — usually a volume, support or skin-quality issue. Many people over 40 have both, because years of movement eventually etch dynamic lines into the skin.
How Doje Clinic uses each
Botox at Doje is used precisely and modestly, aiming to soften expression rather than freeze it. It is also used for structural purposes, such as relaxing a strong jaw muscle (see jaw Botox), and as Skin Botox for pores and fine texture. Filler is placed by layer and purpose: structural filler for deep support and soft filler for delicate or mobile areas. Dr. Byun Jeik, named one of Korea's 22 filler experts by Medical Times, designs both within a single 1:1 consultation so they work together rather than compete.

Side-by-side comparison
| Botox | Filler | |
|---|---|---|
| Mechanism | Temporarily relaxes muscle activity | Adds volume or structural support |
| Depth / target | Specific muscles (or skin, for Skin Botox) | From superficial to deep, depending on gel |
| Best for | Frown lines, forehead lines, crow's feet, jaw muscle | Hollows, folds, flat cheeks, chin, temples |
| Sessions | One; repeat as effect wears off | One; top-ups as advised |
| Downtime | Minimal; small bumps settle quickly | Often minimal; swelling or bruising possible |
| Onset | Typically several days to two weeks | Immediate |
| Longevity | Typically a few months; varies | Varies by product and area |
When Doje combines them
The two are frequently combined because ageing involves both movement and volume. Examples:
- Deep frown lines — Botox reduces the muscle pull, and a small amount of soft filler may soften the etched line that remains at rest.
- Crow's feet with under-eye hollows — Botox softens the lateral lines; soft filler or Eye Rejuran addresses the hollow or thin skin.
- Lower face — Botox can relax a muscle that pulls the mouth corners down, while marionette filler supports the fold.
If sagging is the underlying cause, neither may be enough on its own, and Dr. Byun may suggest a thread lift as the foundation.
Which should you choose?
- Botox — lines that appear only with expression, or muscle-related contour issues such as a square jaw.
- Filler — hollows, folds or flatness visible at rest.
- Both — lines that show with movement and remain faintly at rest.
- Neither — if the main issue is sagging or skin quality; consider lifting or boosters.
Recovery and aftercare
After Botox, avoid rubbing the area and lying flat for a few hours; after filler, avoid pressing or massaging the area. See Botox aftercare and filler aftercare.

Fees are quoted after a 1:1 consultation because areas and quantities vary. Ask for a written quote; Korean non-covered (비급여) aesthetic fees are set per plan.

Frequently asked questions
What is the main difference between Botox and filler?
Botox relaxes muscles to soften expression lines; filler restores volume or support to soften hollows and folds.
Which is better for forehead lines?
Forehead lines that appear with movement usually respond to Botox. Deep lines visible at rest may need additional treatment.
Which is better for nasolabial folds?
Nasolabial folds are mainly a volume and sagging issue, so filler or lifting is usually more relevant than Botox.
Can I have Botox and filler on the same day?
Often yes. Doje plans both in one consultation when each addresses a different problem.
How long does each last?
Botox typically lasts a few months, while filler longevity varies by product and area. Individual results vary.
